Permission Note Sample Wording - LULU.COM

Dear Parent/Guardian,

We would like to publish some text or images by your son/daughter at www.lulu.com, where the work will be available for purchase in the form of a traditional, printed book by anyone with an internet connection.

Our publishing project is designed to encourage students to value the work that they produce and have a sharper awareness of ‘audience’. Perhaps no one will purchase their work, perhaps a few copies will be purchased for family members or as gifts to friends, or perhaps, conceivably, the book will become a popular online purchase and sell many, many copies. The point is, the work the student produces is available to the world. This will likely have an impact on the way the student thinks about what they are achieving school.

There are two issues I think need to be addressed before proceeding with the publication:

1. Copyright.
The student has copyright over their own work, and for NBCS to publish their work, the student needs to give permission. We also seek parent/guardian permission.
It is unlikely but possible that the published work could make a significant amount of money, if it were to become popular and well known. NBCS will put any profits into a charity such as Mission Cambodia.

2. Privacy.
As a general rule, we like students to remain anonymous online until they are old enough to make fully informed and mature decisions about what information or work they will or will not associate with their name in public arenas. This is particularly important in an age where information that is saved to the internet will often be archived an accessible for many years to come. Therefore, we propose that the student is either known only by their first name, year, and school, e.g. “John, Year 3 NBCS”, or by a ‘pen name’ nominated by the student and their parents/guardian.
Please fill in and return the attached form if you and your child are happy to proceed with the publication. Feel free to contact me on 9450 1311 with questions or comments.

Sincerely,

Stephen Collis
